Support Coordination Role
The Support Coordinator role is dedicated to assisting individuals in coordinating and implementing all supports outlined in their funded plans.
* Strengthen the individual's ability to coordinate supports, participate in the community, and ensure the delivery of their plan.
* Build the individual's capacity to achieve greater independence and self-direct their services and supports in the long term.
* Provide timely reports on outcomes and success indicators.
* Develop and implement solutions that enhance the individual's inclusion in their community.

* Knowledge of relevant disability support frameworks and policies.
