We have a new support opportunity available for an upcoming TAC client discharge.
Start Date: Thursday, 16 April 2026 (On - going shift)
Client Requirements:
* Assistance with showering / basin wash
* Support with pressure care and wound care management
Client Status:
* Independent with 2-hourly repositioning (bed/chair)
* Independent with transfers (lie to sit and slide board to/from wheelchair)
* Independent with dressing (upper and lower body)
* Independent with grooming
* Stoma and catheter in place, with Hospital in the Home also involved alongside support services
