**Job Overview**
Hospital Outreach Post-suicidal Engagement (HOPE) is a psychosocial support service delivering outreach and engagement to individuals at risk of suicide.
The HOPE program facilitates services across various domains, including community and social services, housing, finance, family support, cultural and spiritual support, education and employment, and advocacy. Our team provides proactive and sustained efforts to engage with clients in suitable locations, promoting their safety and quality of life.
**Key Responsibilities**:
* Develop professional relationships with at-risk individuals, their families, and other stakeholders.
* Assess referrals and oversee psychosocial support for an ever-changing client base.
* Lead a team of support facilitators while developing and assessing goal suitability and safety plans for clients.
**Required Skills and Qualifications**

Ability to work in a multidisciplinary environment,
Highly organized and adaptable,
Understanding of technical terminology in a clinical setting. A qualification or experience in mental health, social science, or a relevant field is also preferred.
