The Clinical Coordinator plays a pivotal role in elevating workforce capability to deliver exceptional patient care and high-quality support.
By providing staff with necessary skills and guidance, Clinical Support ensures seamless service delivery and optimal client outcomes.
This position requires an experienced nurse who can pivot priorities to meet organisational needs.

Key Responsibilities:
* Implement clinical governance as part of our Disability Support Service Delivery Model.
* Report Clinical Support, quality, and safety performance and risks to senior management.
* Promote a 'Just' quality, compliance, and safety culture and contribute to capability uplift.
* Work as part of a monthly on-call roster.
* Collaborate with clients and stakeholders to address complex clinical needs, support Disability Support Workers (DSWs), and Client Services Managers and Officers.
* Consult with health professionals to provide optimal outcomes for clients by enabling and supporting DSWs.
* Coordinate and ensure completion of clients' clinical assessments, treatment, and support plans.
* Ensure accurate documentation and initiate opportunities to improve client outcomes.
* Operate in a collaborative and consultative manner with clients, their families, medical practitioners, allied health professionals, DSWs, and the wider client services team.
* Ensure client's clinical support plans are being delivered by the DSW workforce.
* Complete audits of clinical processes, identify deficits, and coordinate solutions.
* Monitor standards of care provision and initiate appropriate action if required.

Preferred Qualifications
* Current AHPRA Nurse registration.
* Minimum 5 years clinical experience preferably in subacute services.
* Experience in clinical education – preferably certificate IV in workplace training or equivalent.
* Current First Aid Certificate with CPR.
* Current Victorian driver's licence and own reliable transport.
* Current NDIS Worker Screening Check.
* Satisfaction of all requirements for working in Australia.
* Experience working in a community, residential, and/or rehabilitation environment.
* Awareness of relevant legislation such as the Disability Act 2006 or the Department of Health and Human Services' ISO Standards.
