Aged Care Professional - Manjimup Region

About the Role
This pivotal role will involve coordinating services to meet individual needs, ensuring exceptional client engagement, compliance, and operational efficiency. You'll be the local point of contact for clients, families, and stakeholders, while managing a team of support workers.
Key Responsibilities
* Manage day-to-day operations for aged care home care and NDIS services in the Manjimup area.
* Conduct client assessments and develop person-centred care plans in consultation with clients and families.
* Oversee rostering and scheduling to ensure timely and quality service delivery.
* Coordinate, monitor and review services to ensure alignment with client goals, approvals and funding allocations.
* Act as the primary contact for clients and families, building trust and strong relationships.
* Travel within the region to conduct home visits and provide in-person support.
Required Skills and Qualifications
We're looking for an individual with:
* Certificate III in Aged Care/Community Services (or equivalent).
* Extensive experience in aged care sector, ideally complemented by some exposure to disability, health, or a related sector.
* Strong leadership and coordination skills with experience managing teams.
* Excellent communication and relationship-building abilities.
* Strong organisational and time management skills, with a proactive approach to problem-solving.
