You manage caseloads, build strong connections and work with teams and services to support safe transitions and positive outcomes. Assess needs and develop care and support plans Provide counselling, advocacy and patient support Manage complex caseloads with sound judgement Work with teams to coordinate care and discharge Liaise with community, LHN and service providers Guide and support less experienced team members Maintain clear records and contribute to service improvement About You You are a compassionate, skilled clinician who brings strong judgement, cultural awareness and a collaborative approach to supporting people with complex needs.
