We are seeking an enthusiastic and experienced professional to support our Care Partners and consumers with exceptional customer service.
This fast-paced and highly rewarding role will see you supporting the delivery of consistently high-quality services that meet the needs and expectations of NovaCare customers.
* Supporting Care Partners in providing customer-focused care that exceeds requirements and delivers positive outcomes for consumers.
* Developing and maintaining good relationships with relevant staff to ensure optimum outcomes for consumers.
The successful applicant will be a passionate advocate for exceptional customer service and have a demonstrated ability to build rapport, establish relationships and adapt communication style to suit different audiences.
Key Responsibilities:
* Maintain accurate and timely Care notes that support quality outcomes under the Strengthened Aged Care Quality standards.
* Ensure that service requests are submitted accurately and in a timely manner, working collaboratively with the scheduling team to resolve any scheduling issues or concerns.
* Work collaboratively with Care Partners and Clinically Qualified Care Partners to follow through on care instructions and service coordination.
* In line with escalation protocols, promptly escalate clinical concerns or changes in participant condition to the Care Partner or Clinical Care Partner.
* Identify opportunities for innovation and take personal responsibility for continuous improvement and quality of own work.
* Maintain own professional development in line with the requirements of the role in accordance with current standards.
