Job Title: Chronic and Complex Care Coordinator
This role is crucial in delivering care coordination and management of chronic illnesses to clients and their carers.
The ideal candidate will possess a qualification in nursing, a minimum of 5 years post-graduate experience in Paediatric nursing, and expertise in assessment and management of children with asthma and eczema, discharge planning and care planning.
In this position, you will collaborate with community healthcare organisations to ensure clients receive the right service mix to safely support care in the community.
Providing a safe environment for children and young people involved in our services is a top priority.
