Job Title: Social Worker
We are seeking a dedicated and compassionate social worker to join our team. In this role, you will be working closely with individuals living with complex mental health conditions.
Key Responsibilities:
* This position involves providing ongoing care and support for clients living in the community.
* You will work as part of a multidisciplinary team (MDT) assessing client needs and developing personalized plans for their recovery and well-being.
* Maintain accurate records of client interactions, assessments, and progress notes ensuring that all documentation meets relevant standards.
The MDT delivers comprehensive services across Child & Adolescent, Adult Community, Extended Treatment/Rehabilitation Services (ES), Older Person Mental Health Service areas including an acute 24-7 On Call service based at Royal Hobart Hospital.
