About the Role
We are seeking an experienced and passionate Care Partner to join our growing team. This role is central to the client journey, with responsibility for care coordination, oversight, and budget management for clients receiving services under the Support at Home program.
You will work closely with clients, families, carers, and health professionals to ensure services are well-coordinated, compliant, and aligned with each client's goals.
Key Responsibilities
* Conduct comprehensive assessments and develop person-centred care plans
* Coordinate and oversee service delivery in line with client goals and preferences
* Provide guidance and support to carers and support staff
* Monitor client budgets and service costs
* Build strong relationships with clients, families, and external stakeholders
* Ensure compliance with the Strengthened Aged Care Quality Standards and organisational policies
About You
To be successful in this role, you will have:
* Experience in care coordination, assessments, and aged care compliance
* Strong clinical knowledge, ideally within aged care or community settings
* AHPRA registration as an RN or EN, or an allied health background (highly regarded)
* Excellent communication, rapport-building, and problem-solving skills
* A proactive, organised approach with strong leadership capability
* A genuine passion for supporting older adults to live safely and independently at home
* A current driver's licence
