Complex Care is a community program that sits with in the CART directorate. This program provides care to clients with chronic disease and/or complex psychosocial issues. The community social worker will work with the client and family to improve their overall health and wellbeing. The community social worker will support the Better at Home initiative to reduce avoidable presentations and admission to acute Monash Health services and reduce length of stay when admitted.
We are looking for talented Social Worker Grade 3 for a Part-Time ongoing role. This role requires you to provide a secondary consult and/or comprehensive assessment, planning, coordination of care for clients who have complex health issues/needs in our Community Assessment and response Team - Intensive Management and Support. You will also provide education and support to people with chronic illness to reduce avoidable presentations to hospital.
A Day in the Life
The focus of you as the Senior Social Worker will be providing comprehensive psychosocial assessment, planning, coordination of care and intervention for clients who have complex health issues/needs. You will work in collaboration with the multidisciplinary team and will be responsible for holding a clinical caseload which will involve care coordination, systems navigation, secondary consultation, service development and innovation.
Your aim is to ensure that the clients and families have good community supports and connections. The program utilises an assertive outreach model in line the programs guidelines providing home visits as appropriate to client residing in the Monash Health catchment. You will be required to work in any clinical area at any or all campuses of Monash Health as and when required.
About You
We are seeking a dynamic Social Worker with a genuine passion and interest in this unique opportunity, combined with proven experience and effective clinical/psychological assessment skills. You will also have proven ability to work both independently and collaboratively, and you readily build strong relationships with stakeholders in the delivery of client centred care.
What you need:
1. Bachelor of Social Work or Masters Social Work or equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
2. Previous Community experience (Desirable but not essential)
3. Valid Driver's licence
