What You Will Be Doing
The Community Care Social Worker is responsible for coordinating intensive, short term (3-6 months) outreach care, and delivering allied health interventions within their scope of practice to clients who have chronic health conditions and / or complex health needs.
The role involves facilitating collaborative integrated care planning across acute and community-based services. Care coordinators support people to navigate the service system and services. They provide the point of contact as people undertake their care plan.
What You Need
We anticipate you will have:
• Social worker with current registration with Australian Health Practitioner Regulation Agency (AHPRA) or relevant professional organisation, or eligible for AASW membership.
• Demonstrated ability to manage multiple competing tasks and effectively prioritise.
• Demonstrated experience working with people who are at-risk and vulnerable and have complex needs.
• Specialist experience and knowledge regarding chronic disease management.
