Qualifications & experience
* Bachelor's degree in Social Work.
* Current AHRPA or AASW registration.
* Experience working with individuals with disabilities or behavioural difficulties with a curiosity and knowledge about mental health disorders.
* Ability to work effectively in a team and collaborate with stakeholders.
* Ability to maintain professional boundaries and maintain confidentiality.
* Current South Australian WWCC And NDIS Worker Screening Clearance.
* Current driver's license and willingness to travel to different locations.
* Excellent communication and interpersonal skills.
* Current suitability confirmed by NDIS Quality and Safeguarding Commission at the core or above levels.
Tasks & responsibilities
* Working with clients to achieve their goals in line with this NDIS plan and providing ongoing support coordination services to clients in line with their NDIS plan.
* Using a coaching approach to assist clients in capacity building and maintaining accurate and up-to-date case notes on clients' progress.
* Engaging with clients, their supports and third-party organisations in a professional manner.
* Undertaking work in line with the organisation's policies, procedures and values and complying with all relevant legislation and codes of conduct.
* Maintaining an up-to-date knowledge of the NDIS systems and practices and clear professional boundaries with clients.
* Maintaining an up-to-date knowledge of local services and organisations clients can utilise and engaging and coordinating service providers on behalf of clients and linking clients to relevant providers.
* Meeting with and encouraging clients to develop and refine their goals.
* Engaging clients' informal supports (family, carers, etc).
* Working with customers to help build their own capacity and independence, providing crisis support to clients and assessing and managing risk in line with the organisation's policies and procedures.
* Engaging with NDIA staff and LACs.
