About The Opportunity
As a Senior Social Worker, you will work within a dynamic multidisciplinary team to provide high-quality, evidence-informed social work interventions for clients living in the community. You will play a key role in enhancing patient and carer wellbeing, delivering case planning, counselling, advocacy, and referral services to ensure access to appropriate support networks.
In This Role You Will
* Provide assessment, counselling, and case management to clients experiencing acute and chronic conditions.
* Work collaboratively across specialist programs including Aged Care, Palliative Care, Chronic Disease Programs, and Integrated Care Coordination.
* Supervise, mentor, and provide professional development for Level 1/2 social workers and students.
* Contribute to service development, evaluation, and improvement initiatives within CCLHD Social Work.
* Manage caseloads and ensure timely, person-centred care across community sites.
* Travel throughout the Central Coast to provide care in home, clinic, and group settings.

About You
We are looking for someone who has:
* A degree in Social Work from an accredited course and eligibility for membership with the Australian Association of Social Workers (AASW). Overseas qualifications must include AASW Overseas Assessment number.
* Extensive clinical experience in the community setting across a broad range of conditions.
* Strong skills in staff supervision, team management, and mentoring of students and junior staff.
* Excellent assessment, intervention, and group work skills.
* High-level communication, interpersonal, negotiation, and advocacy abilities.
* Capacity to prioritise competing demands, adapt to dynamic situations, and maintain professionalism in complex environments.
* A current NSW driver's licence.
