West Gippsland Healthcare Group (WGHG) is seeking a dedicated Assessment Officer to join our Cognitive, Dementia and Memory Service (CDAMS) on a fixed term part time basis, from December 2025 to March 2026 (long service leave backfill). This specialist clinic supports individuals with cognitive and memory impairments, along with their carers and families, by providing early assessment and diagnosis.
In this role, you will conduct assessments, coordinate diagnostic clinics, provide education for clients and their family. You'll work closely with a multidisciplinary team to deliver person-centred care and help connect clients with appropriate services.
We're looking for a qualified health professional with experience in community healthcare, strong communication and assessment skills, and a collaborative approach. A current driver's licence and proficiency in Microsoft Office are essential.

Our preferred candidate will have:
* Degree or other qualification in Health Sciences.
* Current valid registration that allows you to fulfil the key responsibilities of this role.
* Excellent Assessment skills and relevant experience.
* Strong understanding of Dementia and the impacts on cognitive assessments
* Responsible and mature work attitude.
IMPORTANT:Some roles in the healthcare sector are required to be vaccinated annually for Influenza (unless a valid exemption applies). Evidence of vaccination (or exemption) will be required to be provided prior to commencement.
