• Work with participants experiencing AOD harm
• To provide effective healthcare nursing support and advice
• To focus on undertaking an internal audit of medication management and systems
• To provide effective supervision of medications and health issues for participants of the program
• To provide secondary consultation to counsellors relating to medications
• To build professional links with local doctors, clinics and other key stakeholders
* Support the implementation of all aspects of the day to day management, health and wellbeing of consumers at The Bridge
* Undertake an internal audit of medication management and systems and provide report to AOD Services Manager as requested
* Work collaboratively with the AOD Services Manager and Senior Clinician to support ongoing commissioning of quality treatment and care services through phases of focused strategic and project planning
* Identify gaps and offer solutions that support best practice outcomes
* Participate in the development of innovation and proven models of treatment and care
* Facilitate health information sessions for clients and on occasion’s with staff
* Program participants are seen regularly and meeting documented
* Provide holistic approach to participants and their experience of the program.
* Active involvement and consultation relating to doctor appointments
* Provide staff medications training on a regular basis
* Medications are reviewed on intake and at regular intervals
* Secondary consultation and medication planning is undertaken with AOD staff
* Participate in the Supervision of Program Participants self-medication
* Manage pharmacotherapy – script dates and changes
* Maintain medical stocks including all first aid kits used at the facility and in vehicles
* Arrange specialist health appointments and follow up as required for residents
* Liaise with AOD Staff in all matters relating to health care of program participants
* Documentation including case notes, court pathology reports are to be accurate and up to date
* Provide client medical files for review at specified intervals
* Prepare and facilitate groups for program participants when required
* Assessment and review of group content ensuring the provision of relevant information.
* Attendance at CCM meetings and General Staff meetings
* Up to date with Work Health and Safety practices and work in accordance with these
* Accreditation processes are explored, adhered to and implemented, providing continuous improvement to program.
