Job Description
We are seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a key member of our staff, you will have the opportunity to make a direct impact on the health and well-being of others.
Your primary responsibilities will include conducting comprehensive health assessments in various settings, including home visits and skilled facility visits. You will work collaboratively with physicians, case managers, social workers, family members, and ancillary medical personnel as appropriate.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. You will also be responsible for conducting adult/geriatric assessments, providing services in a variety of venues, and working closely with Primary Care Physicians (PCPs) on patient education and follow-up.
