Case Management Specialist
You will be responsible for providing high-quality, trauma-informed case management services focused on safety, stability and well-being. This includes working closely with carers, families, Child Protection and allied professionals to develop individualised plans and monitor progress.
The successful candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment. A strong understanding of child development and family dynamics is also essential.
Key Responsibilities:
* Develop and implement individualised plans for children and their families.
* Work closely with carers, families, Child Protection and allied professionals to ensure the best possible outcomes for children.
* Monitor progress and provide ongoing support and guidance to children and their families.
* Provide advocacy and support for children's needs, ensuring that their voices are heard and their rights are respected.
