Here at 360 Health Management we are a regional disability support provider based in Shepparton, with offices in Seymour, Bendigo, and Swan Hill. We are committed to delivering exceptional service to our community, guided by our core values. Founded when Belinda Davies saw a gap in local support that needed to be filled. Now, 5 years later we have a team of over 70 all with an abundance of skills and knowledge all working towards supporting our participants.
The Role:
You will help coordinate supports to participants in line with their NDIS plan. Through strong relationship building you will help develop strong relationships with support worker teams, local organisations, families, and carers and most importantly our participant to understand their capacity and how to help develop further skills and enhance their life!
Key Responsibilities:
•Working with participants to achieve their goals in line with their NDIS plan or those discussed individually with the participant
•Providing ongoing, consistent, and regular support coordination in line with NDIS plan
•Regular contact with participants both in person and on the phone
•Using a coaching approach to assist participants in capacity building.
•Maintaining accurate and up to date case notes on participants progress, requests, and conversations in line with legislation
•Maintaining an up-to-date knowledge of local services and organisations participants can utilise
•Create and monitor participants Service Agreements and schedule of supports
•Maintain and update the participants budgets in all areas to ensure that they can maintain their supports, daily needs and long-term goals
•Refer participants to specialists and liaises with other health professionals in relation to participants’ needs
•Providing crisis support to participants, case noting and communicating with all providers
•Organising monthly Care Team meetings for all participants
Role Criteria:
•Relevant qualification within Social, Community or Disability sector
•Proven relatable experience working within an NDIS field
•Proven experience in a consumer-focused role meeting the needs of complex clients
•Strong time management skills
•Strong MS Office skills
