The Role:
* Conduct comprehensive functional, environmental, and equipment assessments for residents and clients.
* Develop and implement individualised therapy and rehabilitation plans.
* Recommend and prescribe assistive equipment, mobility aids, and home modifications.
* Support residents in maintaining independence, safety, and quality of life.
* Work collaboratively with nurses, physiotherapists, care staff, and families to deliver holistic care.
* Maintain accurate documentation in line with Aged Care Quality Standards and organisational policies.
* Contribute to manual handling, falls prevention, and wellbeing programs
* Complete therapy assessments and respond to clinical referrals
* Plan and implement individual or group therapeutic activities, whilst working with an interdisciplinary team to maintain an effective referral system and coordinate therapy implementation
* Maintain appropriate clinical documentation and information systems
* Contribute to a multidisciplinary program of continuous improvement
* Synthesise information, through observation and written documentation, to provide recommendations that are relationship centred and informed by evidence based best practice
* Undertake a range of assessments that help to understand the holistic experience of the person
The successful candidate will have:
* Current full, unrestricted registration with the Australian Health Practitioner Regulation Agency (AHPRA) as an Occupational Therapist
* Relevant experience in an aged care environment
* Be relatable and able to share knowledge in a way that supports change at all levels
* High level of negotiation, communication and relationship building skills
* Excellent problem solving and decision-making skills
* Able to manage a complex workload and adapt to fast changing priorities and environments
* Is customer focussed and works effectively with a broad range of stakeholders
* Demonstrates effective communication, strong verbal and written skills, and interpersonal skills
