Are you passionate about helping older people live safely and independently at home? UnitingSA is seeking a CHSP CarePartner to join our dedicated Community Aged Care team. What You’ll Do
* Coordinate services that promote independence and support clients in achieving their goals.
* Assist clients to navigate My Aged Care and access appropriate services.
* Collaborate with clients, carers, and key stakeholders to deliver goal-oriented care.
* Co-design and manage care plans inline with My Aged Care assessment of client’s needs and goals.
* Design and manage engaging social group programs.
* Monitor service delivery ensuring it aligns with funding and legislative requirements.
* Escalate high-risk situations in line with UnitingSA policies and procedures.
What You’ll Bring
* Qualification in Aged or Community Support or equivalent qualification.
* Demonstrated experience in aged care or community services.
* Strong communication, organisational, and time management skills.
* Familiarity with electronic rostering systems and consumer databases.
* Understanding of Commonwealth Home Support Program, HomeCare Packages, and Aged Care Quality Standards.
* Proficiency in computing skills and ability to learn new systems.
* Experience with My Aged Care and CALD communities.
* Knowledge of aged care funding programs and electronic care systems.
