The Role
An opportunity has become available for a Nurse Practitioner (Diabetes) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
- Deliver clinical assessment, education, and management of patients with complex diabetes
- Provide advanced clinical management to optimise glycaemic control
- Act as a clinical resource to the multidisciplinary team, providing consultation on diagnosis
- Deliver safe, evidence-based, cost-effective care within the full scope of Nurse Practitioner practice
About you
To be successful in this position you will require:
- Bachelor of Nursing or equivalent
- Registration with the Nursing Midwifery Board of Australia
- Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
- Tuberculosis screening, to identify if the tuberculosis vaccination may be required
- Declaration of serious discipline history must be disclosed
- Criminal History Screening will be required for new employees (GCH will cover cost)

- Base salary ranging from $149 259 - $155 831 per annum.

The Diabetes Service is a community-based program within the Chronic Disease and Post Acute Programs (CD&PAP;), delivering comprehensive diabetes management to adults above 18 years and over. The service operates through a collaborative interdisciplinary model of care, working closely with a range of healthcare professionals to support patients effectively.
This Service is one of the several chronic disease programs within CD&PAP; which includes Cardiac Rehabilitation, Heart Failure, Respiratory Services, Falls & Balance, Chronic Kidney and Peripheral Vascular Disease clinics. The overarching goal of all programs is to maximise medical therapy and empower clients to self-manage their chronic health conditions.
