About the Role
We are seeking an experienced and skilled Care Professional to join our team. As a Care Partner, you will be an integral point of contact for workers and clients, providing support to the broader community.
To succeed in this role, you must hold:
* A formal tertiary qualification such as a Bachelor of Nursing or equivalent
* Current registration as a Registered Nurse (mandatory)
* National criminal history/police check
* Valid First Aid and CPR certificate
* Valid Working With Children's Check Card
Experience and skills required include:
* Knowledge of the aged care industry and issues, including Community Home Care, Continuous Quality Improvement, the Aged Care Quality Principles, and the Aged Care Quality Standards
* Previous experience in Aged Care or a related discipline
* Easily adaptable to new software platforms
Your Responsibilities
You will be responsible for:
* Building strong and meaningful connections with clients and their families
* Conducting comprehensive client assessments and developing care plans to support our clients to remain within their home
* Interacting and providing professional health and clinical care advice and direction to a team of home care workers
* Professionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfaction
* Ensuring all practices are consistent within policy and legislative frameworks
* Maintaining up-to-date client records and information
