About The Role
As a Clinical Nurse Specialist - Acute to Age-Related Care Services, you'll play a key role in achieving our goal of 'healthier rural people & thriving communities'.
You will be responsible for identifying, planning, and facilitating care for older persons after hospital stays. Your duties will include post-hospital monitoring and acute care intervention using a case management model across the continuum of care. The role involves identifying support needs for discharged older persons and working closely with patients who have multiple healthcare needs.
