Role Overview
This role plays a vital part in identifying the needs of older adults and connecting them with essential services to promote independent living.
The successful candidate will conduct thorough assessments to identify the needs of older adults, facilitate referrals to necessary services, and engage with families and carers as key stakeholders.
Key Responsibilities
* Conduct thorough assessments to identify the needs of older adults and facilitate referrals to essential services that support independent living.
* Promote the retention and maximization of function and capability in daily living by engaging with families and carers as key stakeholders.
* Ensure all assessments are conducted in line with contractual requirements.
Requirements
* A minimum of 1 year experience in aged care, including client assessments and/or service delivery.
* Tertiary education in a health-related discipline such as nursing, occupational therapy, physiotherapy, or social work.
* Current unrestricted general registration with AHPRA or relevant professional association.
* Proficiency in Microsoft Word, Excel, Outlook, and client management systems.
* A current Australian driver's license and the ability to travel within your service area.
