About the Role
We are seeking a compassionate and organised Personal Support Coordinator (PSC) to join our team. This role is central to ensuring residents’ personal care needs are identified, supported, and regularly reviewed, while maintaining strong connections with families and allied health professionals.
Key Responsibilities
- Develop, implement, and review individual personal support plans for residents.
- Coordinate daily personal care services in line with residents’ needs and preferences.
- Manage staff rostering to ensure adequate and consistent care coverage.
- Refer and arrange allied health services to support residents’ wellbeing.
- Communicate effectively with residents, their families, and the wider healthcare team.
- Monitor and document changes in residents’ needs and ensure timely updates to care plans.
- Support compliance with the Social Services (Supported Residential Services) Regulations 2024 and organisational policies.
About You
· Experience in aged care, disability, community, or supported residential services.
· Strong organisational and rostering coordination skills.
· Knowledge of allied health and community service referral pathways.
· Excellent communication and interpersonal skills, with the ability to build trust and rapport with residents and families.
· Commitment to cultural safety and inclusive care.
· Relevant qualification in community services, aged care, nursing, or a related field
