About the Role
The primary purpose of the Primary Care Outreach Geriatrician is to provide specialist advice to General Practitioners and other community providers on management of older patients. This will include telephone, videoconferencing and some in-person advice or review of community-dwelling older people with an aim to prevent functional or medical decline. The Geriatrician will aim to reduce decline in function or hospitalisation for frail older people with chronic or subacute frailty or comorbidity.
Selection Criteria
MBBS or equivalent, currently registered or eligible for registration with the Medical Board of Australia. Applicants must have Fellowship of the Royal Australasian College of Physicians or recognition as a specialist by the Medical Board of Australia.

Appropriate experience to practice as a Senior Medical Practitioner in Geriatric Medicine.
Demonstrated experience in community-based Geriatric Medicine, preferably as part of a multidisciplinary community-based team.
Demonstrated excellent interpersonal and communication skills.
Current NSW Driver's licence.
Demonstrated computer literacy skills and knowledge of Microsoft Office and email applications.
