The program is voluntary and can be accessed through self-referral or referrals from trusted parties.
Your Key Responsibilities
* Deliver one-on-one appointments with clients, either in-person or remotely.
* Work collaboratively with clients to identify and set personal goals.
* Facilitate connections with external resources to support goal attainment.
* Develop tailored strategies and skills to enhance client outcomes.
* Provide access to educational and vocational training opportunities.
* Offer financial literacy support, including connection to relevant services.
* Promote social connections and community engagement.
* Establish links with other service providers for housing and employment support.
* Emphasize the importance of holistic well-being.
