The Pharmacy Department, located at Townsville University Hospital are seeking an experienced Pharmacy - Senior to undertake a project role titled Reducing hospital re-admission for high-risk CARDiology patients (RECARD). The project is implementing a pharmacist-led interdisciplinary transition of care service to cardiology patients. It aims to improve patient re-integration from hospital to community and reduce medication-related harm readmissions. About the opportunity Participate in, manage, and lead specialist clinical pharmacy services within the RECARD team. This involves delivering a high quality, evidence based clinical service within a multidisciplinary health care environment to cardiovascular patients, and includes the implementation, delivery and evaluation of a novel transition of care pharmacy service. Your Key Responsibilities: This pharmacist is responsible for providing clinical pharmacy services to patients admitted under the cardiovascular teams. This includes providing a transition of care pharmacy service to identified patients. Registration as a pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA). While not mandatory, a post graduate qualification (diploma or similar) would be well regarded. While not mandatory Clinical experience at Senior Pharmacist level or equivalent, and/or a relevant post- graduate qualification in clinical pharmacy services will be well regarded. Experience in electronic medicines management systems, transition of care pharmacy practice, medicines review, research, and education would be well regarded. The position may be required to work across any of the sites / facilities within the Townsville Hospital & Health service based on current service needs and priorities.
