The Senior Care Services Coordinator will play a vital role in ensuring the best possible outcomes for seniors. This position involves managing the intake of new clients under the Commonwealth Home Support Program (CHSP) and coordinating services to meet their individual needs.

Key Responsibilities:
* Managing the intake of new clients under the CHSP program
* Coordinating services to meet individual client needs
* Providing exceptional customer service to clients, families, and stakeholders
* Assisting with scheduling services
* Guiding and mentoring staff to deliver quality care that promotes client choice and independence
Requirements:
* Strong coordination skills and experience within the CHSP program
* Excellent communication and interpersonal skills
* Cert IV in relevant industry qualification or significant industry experience
* Current unencumbered Driver's Licence
* Current first aid and CPR certification
* Satisfactory Criminal Record Check
* Ability to mentor and share knowledge with less experienced team members
* Capacity to manage a complex workload by planning and organisation of priorities
