Primary Objective: To coordinate and provide an opportunity for clients to reach restorative goals by improving physical, psychosocial and or cognitive functioning. This program aims to reduce the risk of hospital readmission and premature entry into residential aged care by utilising a multi-disciplinary team approach. This position is part of the Aged Care Unit which provide a number of services designed to meet the needs of our ageing population delivering a person centred, wellness and reablement approach.
