Clinical Care Coordinator Opportunity

We are seeking a highly skilled and dedicated individual to join our team as a Clinical Care Coordinator. This is a part-time opportunity that offers flexibility, with the option to work 4 days per week or a 9 day fortnight.

The role involves contributing to high-quality evidence-based clinical care as a member of a multidisciplinary consortia team. You will be responsible for delivering care coordination services for children and youth, focusing on early intervention and support up to the age of 25.

Key aspects of this role include:

* Addressing service gaps for the 'missing middle' by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.
* Promoting holistic wellbeing by considering broader determinants of health, fostering collaboration among providers, and ensuring a sustainable, adaptable service responsive to community needs.

As a Clinical Care Coordinator, you will have the opportunity to make a positive impact in the lives of individuals and families. We are looking for someone who is passionate about delivering high-quality care and making a difference in their community.

This role requires a strong understanding of evidence-based interventions and clinical practice, particularly in the areas of mental health and alcohol and drug use. You must also have excellent communication and interpersonal skills, as well as the ability to engage with people, advocate on their behalf, and establish and maintain effective relationships.

Required qualifications include:

* Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, OR, AASW registration/eligibility as a Social Worker, OR, Level 2 ACA registration/eligibility.
* Experience in the use of electronic client/customer management systems, ability to use a desktop computer, and experience with Microsoft Office applications.
* Eligibility to apply/currently hold a Blue Card for paid employment, and NDIS worker screening clearance for paid employment.
