Your work will directly support clinicians to triage effectively and deliver high-quality care. Receive, assess and process referrals into community, aged care and priority population services Collect, collate and accurately enter demographic and clinical information using systems such as iPM, Cerner and My Aged Care Provide clear, professional information to clients, referrers and health professionals Maintain high data quality through self-auditing and adherence to policies and protocols Collaborate with the team to manage workflow, escalate complex referrals and improve service efficiency How To Apply Apply online by submitting your resume and addressing the criteria listed below by providing examples that demonstrate your ability before the closing date. Selection Criteria High level computer knowledge in data bases or Health information systems such as: iPM, CERNER, and Microsoft Word & Excel applications. Excellent time management and organisation skills with attention to detail & demonstrated capacity to work with minimal supervision and to undertake a diverse range of tasks and problem solve with limited direction. Demonstrated experience in referral pathways, person-centred care and patient privacy. Commitment to ongoing professional development. Commitment to Quality improvement and WH&S responsibilities. Completion of a medical terminology course or equivalent experience in a health-related setting is desirable. Current NSW driver’s license.
