Lead Care Manager Role
The role of a Lead Care Manager is crucial in ensuring the highest standard of ongoing assessment, care planning, and evaluation for residents.
Key Responsibilities:
* Oversee daily clinical operations, including resident care and collaborating with the General Manager to deliver person-centred care.
* Support and lead the care home clinical team, including coaching, mentoring, and performance management.
* Review clinical incidents, complaints, and changes, providing solutions to reduce risks and ensure compliance with relevant assessments.
* Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers.
* Foster a strong team environment, promoting collaboration and open communication among staff.
* Prepare and conduct clinical governance audits, identifying areas for improvement and implementing solutions.
* Liaise with allied health professionals, medical practitioners, and other services to provide optimal resident care.
Requirements:
* Bachelor of Nursing with current registration (essential).
* Postgraduate qualification in aged care (highly regarded).
* Strong leadership, communication, and interpersonal skills, with a passion for nurturing a positive team environment.
* A person-centred care mindset, with empathy for residents and their families.
* Proven track record in delivering quality clinical care within an aged care or acute setting.
* Excellent clinical assessment and triaging skills, as well as strong care planning capabilities.
* Understanding of Resident Rights and Aged Care Accreditation Standards.
