* Due to continued growth, DSN National is expanding and is currently recruiting an experienced NDIS Level 2 Support Coordinator to join our dedicated team.
Role Purpose
* The NDIS Level 2 Support Coordinator will work alongside participants to build their capacity to understand, navigate, and implement their NDIS plans.
* he role supports participants to connect with appropriate services, strengthen informal and formal supports, manage service delivery challenges, and work towards their individual goals.
* This position requires a minimum of two years' experience delivering Support Coordination within the NDIS and a strong commitment to quality, ethical, and participant‐led practice.
Key Responsibilities Participant Support
* Work collaboratively with NDIS participants to implement and maximise their NDIS plans
* Build participant capacity to understand their funding, supports, and service options
* Support informed decision‐making, participant choice, and control
* Identify risks, barriers, and emerging needs, and respond proactively
* Assist participants during plan implementation, reviews, and transitions
Coordination & Collaboration
* Source, connect, and monitor appropriate service providers aligned with participant goals
* Develop and maintain strong working relationships with participants, families, carers, providers, and community networks
* Advocate on behalf of participants when required, while promoting independence and self‐advocacy
* Coordinate complex and multi‐service supports effectively and ethically
Documentation & Compliance
* Maintain accurate, timely, and professional case notes and documentation
* Ensure all work aligns with NDIS Practice Standards, Code of Conduct, and organisational policies
* Monitor service delivery quality and address concerns or gaps promptly
* Contribute to reports and documentation for NDIS reviews and audits as required
Professional Practice
* Participate in supervision, professional development, and team meetings
* Maintain current knowledge of NDIS guidelines, legislation, and best practice
* Uphold confidentiality, integrity, and a strengths‐based, trauma‐informed approach
Essential Criteria
* Minimum 2 years' experience delivering NDIS Level 2 Support Coordination
* Strong understanding of the NDIS, including planning, funding categories, and service pathways
* Excellent communication, organisation, and problem‐solving skills
* Ability to work independently and manage a diverse caseload
* Must be a Team player
* Demonstrated commitment to participant‐centred, community‐based support
* Competency with electronic case management systems and documentation
* Current NDIS Worker Screening Check (or willingness to obtain)
* Current Working With Children Check (or willingness to obtain)
* Valid driver's licence and access to a reliable vehicle
Desirable Attributes
* Relevant qualification in Community Services, Social Work, Disability, Health, or a related field
* Experience supporting participants with complex needs
* Knowledge of local and national community service networks
