Title: Client Planner
Employment: Full Time / Ongoing
Salary: $80,000 p.a. plus super plus salary packaging
Location: Surrey Hills, VIC or Hobart/ Dowsing Point, TAS

As a Client Planner, you'll work closely with clients, families, support coordinators, and service teams to design and review support plans that truly reflect each person's goals, preferences, and strengths. You'll bring person‑centred practice to life, ensuring every plan is aligned with NDIS standards, organisational policy, and the unique goals of each individual.

Working under the guidance of the Client Planning Manager, you'll contribute to high‑quality planning processes across the organisation and help ensure clients receive effective supports.

Your efforts will directly contribute to maximising client funding and achieving their personal goals.

This role requires collaboration across various internal departments, including Finance, Operations, Quality, and Rostering, making it an excellent opportunity for those who thrive in a team‑oriented environment.

Here's what we are looking for:

* Strong understanding of the NDIS, disability services, and person‑centred planning approaches.
* Experience in client planning, case management, or support coordination within the disability, aged care, or community services sector.
* Familiarity with NDIS guidelines, funding structures and compliance requirements.
* Excellent interpersonal and communication skills, with the ability to engage respectfully with clients and families.
* Strong organisational and time management skills, with the ability to balance competing priorities.
* Good problem‑solving and critical‑thinking skills.
* High attention to detail and accuracy in documentation.
* Ability to work independently and as part of a collaborative team.
* Commitment to inclusion, and empowering people with disabilities.
* Relevant tertiary qualifications in social work, psychology, human services, or related field (desirable).
* Previous experience in disability services, aged care, health, or a similar sector (desirable).
