The Role:
* Conduct initial assessments and ongoing interventions for NDIS and Home Care Package participants in clinic or at the participant's home, across the lifespan including complex cases.
* Assistive equipment prescription and minor home modification recommendations. (Experience and knowledge in major home modifications is an advantage).
* Provide quality clinical opinions and justification through comprehensive report writing and completing functional capacity assessments.
* Liaising with service providers and within the team to ensure effective communication.
Key Responsibilities:

2. Conduct thorough assessments and evaluations to identify client needs and goals.
3. Collaborate with multidisciplinary teams to provide holistic care and support.
4. Maintain accurate records and documentation of client progress and outcomes.
