Supporting and enabling patients to achieve their goals and live their best lives, you will provide services across the following packages and programs:Home Care Package clients: supporting independence at home through:Home assessments, equipment prescriptions, and home modifications.Rehab in the home program: focusing on post-orthopaedic surgery recovery.Hospital in the Home program: supporting clients after hospitalisation, including general surgeries.About youWhether a new graduate or an experienced clinician, you love making a difference in Australians' lives. Fully AHPRA registered or Limited Registered (remote level), ideally (but not essential), you will have at least 1 year's experience in a community or aged care setting or be willing to learn.
