About you: In your new role you will * Provide expert guidance, leadership and advice to all directorates of the CHHHS in relation to NDIS processes, NDIA and community disability networks and length of stay issues as impacted by complex needs, and provide independent, high level, specialist clinical Occupational Therapy case management and consultancy as required. * You will utilise high level communication to build and maintain effective relationships with a range of diverse key stakeholders, including the National Disability Insurance Agency (NDIA) and associated community partners, NDIS Providers across the disability sector, Office of the Public Guardian (OPG), as well as CHHHS senior managers, treating teams, people with a disability and their families/carers, and any other relevant internal and external key stakeholder. About your opportunity: In this position you will * Lead professional, patient safety and service improvement activities in the area of NDIS practice and initiatives including the development and evaluation of CHHHS policy, procedures, guidelines and training / education initiatives including oversight of statewide clinical standards for best practice. * Advocate for consumer involvement to provide opportunity for real and meaningful choices about their care and improved discharge plans to assist with long admissions and / or hospital avoidance strategies. * Provide professional supervision, mentoring, consultancy to a range of CHHHS staff, including Occupational Therapists, and support an interdisciplinary practice framework to ensure NDIS practice and interventions are evidence based. About your role: * Provide high level support to teams who work with patients of CHHHS living with a disability who may be eligible for NDIS or are currently a recipient of a NDIS package and will require a NDIS plan/package or review of packages to ensure efficient, timely and safe discharge. * Develop and actively contribute to building the service capability within CHHHS to address the NDIS service needs of patients by providing clinical and strategic direction to clinicians and teams across the CHHHS, including systems and processes, to optimise outcomes for patients who may present at risk of having prolonged hospital stays due to complex needs.
