What you'll be doing in this role?
The responsibilities of a Care Partner are varied and each day brings new challenges. As a member of a larger team, you will oversee a portfolio of clients, ensuring that you provide the necessary support for them to safely stay at home while working towards their goals.
To Be Successful, You Will Require
* Assessment skills: conduct thorough assessments of individuals' needs, including physical, emotional, and social well‐being.
* Care Planning: develop comprehensive care plans tailored to each individual's unique needs and preferences.
* Consumer‐Focused: place the consumer at the center of your work, prioritising their choices, preferences, and goals.
* Liaison with Health Professionals: communicate and collaborate with doctors, nurses, therapists, and social workers to implement and adjust care plans.
* Visiting Consumer: visit consumers in their homes to assess well‐being and progress of their care plan.
* Empathy and Communication: possess strong empathy and communication skills; listen actively and build trust.
* Problem‐Solving and Advocacy: identify challenges and find solutions; advocate for consumer rights.
* Organisation and Time Management: manage multiple cases, prioritise tasks, and meet deadlines effectively.
* Compassion and Patience: provide reassurance, encouragement, and emotional support.
What We're Looking For
* Appropriate qualifications compliant with Australian Qualifications Framework (e.g., Diploma of Case Management and/or Community Services / Nursing / related field).
* A current Victorian Drivers Licence.
* A current Working with Children Check (Employee) or VIT Registration.
