About the role
* Deliver comprehensive social work services to patients, families and carers in the medical/surgical wards at St Vincent's Hospital, providing high-quality psychosocial care within a multidisciplinary team, and participate in the 24-hour on-call social work roster as required
* Conduct psychosocial and risk assessments, contributing to safe discharge planning including community referrals and accommodation in supported living and residential aged care facilities.
* Provide counselling and therapeutic support to individuals and families, assisting adjustment to medical conditions and lifestyle changes
* Advocate for patients and carers, including preparation of applications to NCAT (Guardianship Division) and the NDIA (NDIS) where required
* Facilitate family conferences and actively participate in multidisciplinary meetings to support collaborative, patient-centred care
* Participate in supervision, quality improvement, education and research initiatives.
About you
* Hold a degree in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW), with less than 9 months post graduate qualifying experience (at time of application)
* Be able to, under direct and indirect supervision, conduct psychosocial assessments for patients and assess issues of risk. Understanding of crisis intervention, short-term solution-focused approaches, and grief and loss support.
* Participate in multi-disciplinary meetings for patient care and safe discharge planning including referral to community services, short- and long-term accommodation/ placement for patients.
* Excellent time management and prioritisation abilities, alongside proficient computer literacy and strong written and verbal communication skills.
* Participate in supervision, professional development and quality projects. Complete documentation according to professional standards, provide clinical handovers and maintain statistical records. Develop and maintain resources pertaining to clinical practice.
* Support family conferencing to enable information exchange between the multi- disciplinary team and patients, families and carers.
* Willingness and ability to participate on the 24 hour, 7 day a week on-call service as required.
