Mental Health Nurse - Care Coordinator Role
We're seeking a compassionate and experienced Registered Nurse to drive care coordination for individuals navigating acute mental health challenges.
About the Role
This role requires you to deliver high-quality, patient-centered care through comprehensive assessments, assertive outreach services, and collaboration with multidisciplinary teams.
* Key Responsibilities:
* Deliver triage, comprehensive assessments, and assertive outreach services ensuring clients receive the least restrictive and most effective treatment options.
* Collaborate with Access team and both inpatient and community-based services to co-create treatment plans with clients and their support networks.
* Play a vital role in empowering clients, families, and carers through education and support extending your impact to the broader community.
About You
To be successful in this role, you will bring:
* Relevant tertiary qualifications as a Registered Nurse (including completion of a new graduate RN program or equivalent experience working as a registered nurse) and currently registered with AHPRA
* Demonstrated experience in the assessment and management of people presenting with acute mental health disorders
* Demonstrated excellent negotiation skills and verbal and written communication skills
