This role coordinates the delivery of patient assessment, clinical review, treatment, care planning, and transfer of care for those accessing the service. The NUM mentors, coaches, and supports the nursing and clinical support team to ensure high standards of care and service delivery are consistently met.The NUM 1 plays a key role in facilitating smooth care transitions and ensuring timely escalation to in-person services or alternative providers when required. A strong focus on patient safety, evidence-based practice, and continuous quality improvement is essential. The position also involves close collaboration with internal and external health providers to ensure seamless, person-centred care across both virtual and face-to-face settings, while supporting the integration of clinical pathways including Virtual Urgent Care, Central Intake, Early Supported Discharge, and Aged Care Outreach Services.We’re looking for a collaborative, forward-thinking nurse leader who is ready to be actively involved in clinical operations, service planning, and team leadership.
