Job Overview
We are seeking dedicated professionals to join our team in Victoria, where they will be responsible for providing high-quality physiotherapy services to clients.
Key Responsibilities:
* Aged Care Assessments: Conduct comprehensive assessments of patients' physical abilities and create personalized care plans to address their needs.
* Care Plan development, regular evaluation and incident analysis, Care Plan modification and progression: Work closely with multidisciplinary teams to develop and implement effective care plans that meet patients' changing needs.
* In-service education: Provide training and education to healthcare professionals on best practices in physiotherapy.
* Physiotherapy treatments: Deliver evidence-based physiotherapy interventions to patients, including manual therapy, exercise prescription, and pain management techniques.
* Physiotherapy assessments: Conduct thorough assessments of patients' physical functioning and identify areas for improvement.
* Exercise prescription and the formulation of Care Plans: Develop customized exercise programs that promote patient mobility, strength, and function.
* Implementation of a pain management clinic: Design and lead clinics focused on managing chronic pain in patients.
* Correspondence with and liaison for residents, families, nursing, medical practitioners, allied health practitioners and equipment suppliers: Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
* Facilitation of falls prevention workshops, manual handling and other staff education: Organize and deliver educational sessions on falls prevention, manual handling, and other relevant topics to promote safe working practices among healthcare staff.
