Opportunity to establish yourself in a new role within a supportive team Career growth through ongoing learning and development What you'll be doing Provide optimal service to patients/clients and carers, promoting best practices including care escalation and facilitating patient discharges via the discharge/transit lounge. Assist under the guidance of a Registered Nurse to deliver safe, compassionate, person-centred care, collaborating with the multidisciplinary team to achieve quality outcomes. Communicate effectively with team members to provide discharge instructions, medications, and paperwork to patients/clients and carers. Advocate for the specific needs of patients/clients and carers, including those from Aboriginal & Torres Strait Islander and multicultural communities. Maintain professional standards through ethical practice, adherence to policies, procedures, and relevant legislation. Manage resources efficiently relevant to the service. Exercise autonomy in decision-making regarding patient care and discharge processes.
