About Kimberley Renal Services
Kimberley Renal Services includes 4 Renal Health Centres based in Fitzroy Crossing, Broome, Kununurra, and Derby, as well as a mobile prevention unit. The incidence of Kidney Disease in the Kimberley is one of the highest in Australia.
Chronic Kidney disease (CKD) and End-Stage Kidney Disease (ESKD) incidence within the Aboriginal population of the Kimberley greatly exceeds the national burden of disease. Dialysis prevalence for this region has more than tripled in the last decade and is increasing at a much faster rate than in the rest of Western Australia (WA).
About the Opportunity
This exciting opportunity allows you to join a multidisciplinary renal team and advocate for the rights of renal clients and their family. You will work to establish and implement effective care coordination services within the region to support Aboriginal patients to seek proactive chronic disease management and care.
You will also provide relevant information and education about renal illness and treatments and assist clients to access services. Your key responsibilities include:
* Engaging with Aboriginal renal patients in outreach home settings
* Increasing cultural awareness for renal staff new to the area of Aboriginal health
* Developing and implementing strategies to improve access to mainstream primary healthcare for Aboriginal and Torres Strait Islander people
About You
To be successful in this position, you must be committed to the philosophy and practice of Aboriginal Community Control and have an understanding of Aboriginal culture and a willingness to create a culturally safe environment. A current 'C' Class Driver's Licence is required, and your employment will be subject to a National Police Clearance.
