The successful candidate will be responsible for delivering exceptional client services, managing care plans, and leading support workers to provide outstanding customer experiences.
To succeed in this role, the ideal candidate will have demonstrated experience in community services coordination or case management, with a relevant qualification. They will also possess strong communication skills, the ability to work effectively in a multidisciplinary team, and knowledge of OH&S legislation relevant to the sector.
Key responsibilities include client relationship management, care plan review and management, leading and coaching support workers, service level monitoring, and conducting case conferences with internal and external stakeholders.
