As we continue to grow, we are seeking a highly skilled and experienced Clinical Nurse Specialist to join our team and lead the delivery of complex clinical care in the community.
What We're Looking For
Candidate who can commit to a minimum 20-hour work week, plus the option for additional hours
Current AHPRA registration as a Registered Nurse
Minimum 3 years of clinical nursing experience, with demonstrated capability in community or aged care settings
Strong experience in wound care, catheter care, and complex case management
Proven clinical leadership and mentoring skills
A current driver's licence and access to a reliable vehicle
Current National Police Check or willingness to obtain
Unrestricted work rights in Australia"

Key Responsibilities
Complex Wound Care
Conduct thorough wound assessments (including photography, measurements, and staging)
Develop and implement individualized wound management plans
Evaluate dressing types, advanced therapies, and negative pressure wound options
Insert, maintain, and troubleshoot indwelling (IDC) and suprapubic (SPC) catheters
Educate patients and carers on proper catheter care, infection prevention, and early signs of complications
Patient Assessment & Care Planning
Perform holistic in-home assessments (covering physical, psychosocial, and environmental factors)
Develop and review person-centered care plans in collaboration with allied health and medical teams
Coordinate referrals to relevant health and community services
Clinical Leadership
Mentor and support junior nurses and student placements in the community
Lead and contribute to the development of clinical policies, procedures, and best-practice guidelines
Participate in clinical audits and quality improvement initiatives
