About The Opportunity
South Western Sydney Local Health District is seeking a dynamic and experienced health professional to lead the newly established Community Older Persons Intervention and Liaison Outreach Team (COPILOT). This innovative service delivers person-centred care to older people in the community, reducing hospital admissions and supporting safe, early discharges.
The Team Leader will provide operational and professional leadership and management of staff within the Acute and Extended Care in the Community Directorate, implementing an integrated model of care that is evidence-based and responsive to the medical and physical deterioration of older people.
This role is pivotal in developing and refining the COPILOT service, ensuring optimal service delivery through monitoring activity data, coordinating multidisciplinary care, and fostering collaborative partnerships.

* Must hold relevant tertiary qualification and either be registered with Australian Health Practitioner Regulation Agency (AHPRA), or where AHPRA is not required, be eligible for membership with the relevant professional association.
* Extensive post graduate experience with community, outpatient and/or people with chronic diseases, including a demonstrated understanding of the health setting, complex health conditions and the impact on wellbeing.
* Advanced clinical knowledge and postgraduate experience and skills in aged care assessments and interventions to optimise health and quality of life for patients, carers and families, including identifying and responding to Older Persons abuse.
* Demonstrated experience in prioritising a challenging and demanding workload; managing stressful situations; and applying advanced reasoning skills and independent professional judgement when dealing with situations of a novel or critical nature
* Experience working independently and collaboratively within a multi-disciplinary team
* Demonstrated high level verbal, written and interpersonal communication skills, and a demonstrated ability to negotiate and manage conflict as well as maintain a positive workplace culture
* Demonstrated experience in the provision of clinical supervision, expert advice/consultation within the clinical specialty and leading a team
* Demonstrated experience in the planning, implementing, leading, and evaluating of quality improvement activities involving the application of evidenced based practice within aged care.
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
