**Your new role**
Reporting to the Registered Nurse, in this newly created role you will provide high quality physical and emotional support to clients within community and residential settings. You will also:
- Coordinate and conduct training and educational sessions for staff and community.
- Perform mental health assessments and provide tailored care plans to clients.
- Ensure all care plans are current and compliant with NDIS and other relevant standards.
- Report, escalate and manage all incidents and concerns in a timely manner.
- Assist in community-based initiatives aimed to promote mental health education, reduce stigma, and improve the well-being of clients.
**What you'll need to succeed**
Previous experience providing and coordinating individualised care plans and support within the Mental Health or Disability field. You will also have:
- Bachelor qualification in Nursing, Allied Health or equivalent.
- Current driver's license and reliable transport.
- Strong working knowledge of the Mental Health Act SA 2009 and NDIS legislations and standards. Ensuring all care plans are current and compliant with relevant standards.
- Excellent triage and interpersonal skills. Ability to work as part of a multi-disciplinary team.
